CONSENT
to the collection and processing of personal data

For now, I am,_________________ I provide my unconditional consent to the Public Association "Professional Association of Independent Experts of the Subsoil of PONEN" (hereinafter - NGO "PONEN") for the collection and processing of my personal data, in ways that do not contradict the legislation of the Republic of Kazakhstan, both on paper media and and in electronic format, to the extent necessary. Personal data - information related to me, recorded on electronic, paper and (or) other material media, including, but not limited to: information necessary to fill out the questionnaire, as well as proper identification: surname, first name, patronymic, citizenship; data of the identity document; IIN; • information necessary to keep in touch: place of residence and actual location; place of work and position; phone number (home, work, cell); information about education, professional activity, official position, business reputation: education, profession, qualifications (advanced training, position, academic degree, academic title, knowledge of foreign languages, membership in professional chambers/organizations; information necessary to confirm work activity: data on previous and current jobs, positions held, including, data that can be obtained from documents confirming employment; • information necessary to fulfill the obligations of the NGO "PONEN" to pay taxes and other deductions provided for by the legislation of the Republic of Kazakhstan, including numbers/details of bank accounts, payment cards; • information about state and departmental awards, honorary and special titles, incentives; • information about changes and (or) additions to the above data and other information. This Agreement is valid during the term of membership in the NGO "PONEN", concluded between me and the NGO "PONEN", as well as after its termination for 5 (five) years in order to comply with the requirements of the current legislation of the Republic of Kazakhstan. I hereby confirm that I will not have any claims against the NGO "PONEN" regarding the collection and processing of your personal data in the future, provided that the NGO "PONEN" complies with the requirements of the Law of the Republic of Kazakhstan "Personal Data and their protection".
